To earn QUAD A accreditation, an ambulatory surgery center must pass an exhaustive onsite survey that verifies hundreds of criteria—from emergency‐power backups and sterile processing to medication management and anesthesia protocols.

Ongoing patient-safety reporting

Accreditation isn’t a one-time test. QUAD A requires facilities to submit quarterly Patient Safety Data Reports that track every surgeon’s cases and any unanticipated events.

 Proven safeguard against “hidden” risks

Questions often arise about how outpatient or ambulatory centers are overseen. While every state health department regulates these facilities, nearly half—including New York—require independent accreditation from one of four national bodies. Among them, QUAD A (formerly AAAASF) subjects a center to a 162-page checklist that mandates, for example, a circulating nurse in both the OR and recovery suite, board-certified surgeons, on-site crash carts, and a formal transfer agreement with a nearby hospital.

QUAD A’s past president, Dr. Geoffrey Keyes, notes that those safeguards make death rates in accredited ambulatory centers comparable to hospitals.
